New Patient Information

What Should I Bring to My First Visit?

If you are a member of an HMO, bring your referral from your primary care physician, your photo ID, your insurance card, a list of medications you are taking, copies of labs, x-ray, MRI, CT scan, operative reports, or pathology results that may be relevant to your symptoms, your copayment, and your completed Patient Information form and Health Questionnaire.

View our detailed Financial Policies. If you have a PPO, please note that we collect all applicable copayments, unmet deductibles, and co-insurance at the time of your visit. We verify your benefits before, or when, you arrive and can inform you of your estimated financial obligation at the time of your visit.

We would appreciate your arrival 15 minutes prior to your appointment time for this first consultation visit in order to make sure all your paperwork is completed. If you need to cancel your appointment, we would appreciate advance notification of 24 hours so that your appointment time can be offered to others in need. In order to meet our goal of providing high quality care and convenience for our patients, if you cannot arrive within 15 minutes of your appointment time, please call our office so that alternative arrangements can be made.

  • Turn right off the exit and proceed 1.9 miles
  • Make a U-turn on Ygnacio Valley Road at San Carlos Drive
  • We are located next to the gas station on the right

From the North:

  • Take I-680S to the Treat Blvd/Geary Rd. exit
  • Turn left onto North Main Street
  • Turn Left onto Treat Blvd. and proceed 1.1 miles
  • Make a slight right onto Bancroft Rd. and proceed .6 miles to Ygnacio Valley Road
  • We are located next to the gas station on the right

From the South:

  • Take I-680N to the Crow Canyon Exit
  • Turn right off the exit and proceed .3 miles on Crow Canyon Road
  • Turn right at Camino Ramon and proceed .2 miles
  • Make a left into the 4th driveway on the left, which is the entrance to Bishop Ranch 11.
  • We are located in the far right hand corner as you enter the parking lot.
  • We are in Suite 200 of the 2305 building
  • If you see John Muir Urgent Care, we are located directly above their offices

From the North:
