If you are a member of an HMO, bring your referral from your primary care physician, your photo ID, your insurance card, a list of medications you are taking, copies of labs, x-ray, MRI, CT scan, operative reports, or pathology results that may be relevant to your symptoms, your copayment, and your completed Patient Information form and Health Questionnaire.
View our detailed Financial Policies. If you have a PPO, please note that we collect all applicable copayments, unmet deductibles, and co-insurance at the time of your visit. We verify your benefits before, or when, you arrive and can inform you of your estimated financial obligation at the time of your visit.
We would appreciate your arrival 15 minutes prior to your appointment time for this first consultation visit in order to make sure all your paperwork is completed. If you need to cancel your appointment, we would appreciate advance notification of 24 hours so that your appointment time can be offered to others in need. In order to meet our goal of providing high quality care and convenience for our patients, if you cannot arrive within 15 minutes of your appointment time, please call our office so that alternative arrangements can be made.
