How do I schedule an Office Visit?
Please call (925) 933-8462 and Press 2 to speak with a Walnut Creek Receptionist to schedule your next appointment.  If you’d like an appointment in San Ramon, please call (925) 242-0647 and Press 2.


If you know your insurance requires a referral from your primary care physician (all HMOs require one), please contact your primary care physician and request it from them.  They can fax us the referral directly to: (925) 933-4460 (Walnut Creek) or (925) 242-0618 (San Ramon).  We require a referral in order to schedule all procedures and visits.

If you are a member of an HMO, bring your referral from your primary care physician, your photo ID, your insurance card, a list of medications you are taking, copies of labs, x-ray, MRI, CT scan, operative reports, or pathology results that may be relevant to your symptoms, your copayment, and your completed Patient Information form and Health Questionnaire.  Click here for a New Patient Packet that can be completed online. 

We would appreciate your arrival 15 minutes prior to your appointment time for this first consultation visit in order to make sure all your paperwork is completed.  If you need to cancel your appointment, we would appreciate advance notification of 24 hours so that your appointment time can be offered to others in need.  In order to meet our goal of providing high quality care and convenience for our patients, if you cannot arrive within 15 minutes of your appointment time, please call our office so that alternative arrangements can be made.


Who may I call if I have a question prior to my surgery or procedure?
You can contact our Patient Care Coordinators by calling (925) 933-8462 (Walnut Creek) and entering Option 3. 

 
Who may I call if I have a question about my surgery/procedure results?
If you do not have a Post-Op appointment scheduled or would like to speak with your physician prior to the appointment, please contact our office at (925) 933-8462 (Walnut Creek) or (925) 242-0647 (San Ramon), and press 2 to speak with the Receptionist to leave a message for your physician.  Your physician or our Nurse Practitioner will call you back. 


How do I pay for my visit?

Who may I call if I have a question about my bill?
You can contact our Billing Department by calling (925) 933-2015.
 
May I be billed for my copayment?
Your insurance carrier requires that you pay your copayment at the time of service.  We accept Check, Cash, VISA, MasterCard, Discover, American Express, or Debit card.  (Please note that there is a $35.00 service charge on all returned checks.) 
 

Is there a charge to have my forms completed (e.g. disability)?
There is a $35 charge for the completion of forms such as disability or worker’s compensation forms since these forms requires our doctors’ time for review and completion.


How do I obtain my medication refills?
Call your pharmacy to request a renewal of your prescription.  They will contact us to renew your prescription.  We may request that you come in to see your physician.  Please request your refill at least 3 business days before your prescription runs out.  More insurance companies are requiring pre-authorization of medications so please allow us adequate time to receive approval for your renewals.  If you have any questions regarding medication instructions, please contact the office at (925) 933-8462 (Walnut Creek) or (925) 242-0647 and press 2 to speak with a Receptionist.


Who should I contact if I have a questions about my medications before my procedure?
Contact your physician if you have any questions about your medications. 

 
How can I obtain a copy of my medical records?
Complete the
Authorization for Release of Information and mail, fax, or bring it to the office.
